14.4.1 Prep surface 14.4.1.1 Contractor shall provide all equipment, material, supplies, traffic control, and labor as required. 14.4.1.2 Contractor is responsible for obtaining any permits or certification required by city of Heber. Any permits acquired shall be turn into Housing Authority upon completion of the job. 14.4.1.3 Contractor shall coordinate with maintenance personal and tenant to schedule work demo and installation. Once a schedule has been determined the contractor shall submit the schedule to contracting officer. 14.4.1.4 Contractor will coordinate with Dig Alert for marking of existing lines prior to demolition. (Contractor will be held responsible for any damages cause to any lines underground during the duration of the contract) 14.4.1.5 Perform preparation and cleaning, and safety procedures in strict accordance with the manufacturer’s instructions for the particular substrate condition. 14.4.1.6 Contractor shall demo, saw cut, and remove approximately 1,288 sq. ft of concrete slab in front of units 1-4 as shown on diagram 15.2 on page 12. 14.4.1.7 Contractor excavation and grading minimum of 4” of dirt or as necessary to ensure proper slope and water flow direction away from the house. 14.4.1.8 Dispose of all debris resulting from work on a daily basis. 14.4.2 Installation 14.4.2.1 Install 4” crusher fines or class II base, moisture, grade and compact dirt bottom. 14.4.2.2 Install wire mesh prior to pouring concrete. Contractor shall notify the Contracting Officer to inspect form and mesh installation. 14.4.2.3 Install 4” thick 4,500 psi concrete with thicken edge which will slope away from unit using a flume style concrete sidewalk as shown in diagram 15.3 on page 12. Direct water toward the grassy area on south side of the property. Must meet local building and stormwater codes. Install control joints. Concrete must have a broom finish. 14.4.3 Finished work 14.4.3.1 Final Adjustments: Clean & inspect prior to date of inspection for acceptance. Contractor is responsible for correcting any and all defects. 14.4.3.2 Contractor must notify IVHA Contracting Officer when work is completed to schedule walk through inspection for acceptance of work. 14.4.3.3 The Job Completion Form must be submitted to IVHA along with invoice, payroll, and any certificates or permit obtained to be deemed complete.
