Preemployment Physicals and Drug Screening Services Include: Physical Examination with a Weightlifting Assessment. Drug Screenings to be 10 panel testing from urine sample. Negative drug test is to be sent automatically to facility point of contact. Inconclusive drug tests are sent to lab for confirmation and review. Physical Examinations to be completed to the most recent standard for employment. Lift test to be completed in intervals of 25 and 50 pounds 3 repetitions each. Authorization for all services must be completed by the facility prior to any examination or testing is completed. Authorization for services can be submitted via electronic platform, paper form, fax, or email. Vendor/Staff shall maintain all necessary licensing & certification required by Federal & State Guidelines Vendor is to provide and utilize own equipment and supplies for any service rendered.
