● Smoothies are to be delivered on days and times requested by the ten secondary schools in the Rockwood School District. If a decision is made to occasionally offer 4oz smoothies to our 20 elementary school locations, a mutually agreed upon schedule for deliveries to these locations would be made. A district map of the school locations is enclosed. ● A legible numbered invoice must accompany each delivery with the name of school, date of delivery, number of smoothies and total cost. Rockwood Child Nutrition will be charged only for the smoothies that are sold, not the total delivered. A monthly statement must be provided to Rockwood Child Nutrition listing all deliveries and charges for the month. ● All smoothies covered under this contract are to be delivered under the most sanitary conditions. Smoothies will be delivered in ice chests to each school. The ice chests will be picked up by the vendor at the end of the lunch period. ● Rockwood Child Nutrition requires a copy of the most recent sanitation report. ● Smoothies are to be custom blended with real fruit and fruit juices. They must be made with 100% natural ingredients. No protein powders, supplements or sugar to be added to smoothies. ● Provide a list of flavors and the nutrient analysis for each flavor with bid. ● Quote your price for a 12 oz. and 4 oz. smoothie, served in your cup with lid and straw. Each smoothie must be labeled with the flavor, all ingredients, including water and ice (from greatest to least), production address and allergens.
