The City of Chicago is issuing a Request for Proposal (RFP) to identify qualified organizations to administer an Independent Living Program (ILP) that empowers individuals with disabilities to continue living in their own homes. The selected respondent will be responsible for coordinating and delivering personalized in-home assessments designed to enhance independence, accessibility, and safety for people with disabilities within their own residence. The respondent will be responsible for all aspects of the program, including screening for eligibility of services, intake, assessments, case management, coordinating purchase of assistive devices, and providing other resources and information as identified through the process.
