The Contractor shall provide the following services: A. STEAM CLEANING 1. Contractor shall perform professional steam cleaning of four (4) kitchen exhaust hoods, including all internal and external surfaces, access doors, filter frame and filters, duct work and three (3) roof mounted exhaust fans located at the Main Kitchen of the facility locations. 2. The Contractor shall clean the kitchen hoods as stated in Section 2 above, on a semi-annual basis, not to exceed six (6) months between services on the following schedule: a) October 2025 b) April 2026 c) October 2026 d) April 2027 e) October 2027 f) April 2028 3. Upon completion, Contractor shall provide a Service Report for each hood to the CM or designated representative of the service provided. The report shall include: a) Location of equipment, equipment make, model and, serial number b) Date of service c) Description of service provided d) Description of any noted deficiencies and suggested corrective actions needed or provided e) Name and signature of service technician f) Total labor hours expended g) Signature of State employee certifying indicated services were performed
