Category A: Raise Consumer Awareness: Raise consumer awareness about the importance of financial, legal and advanced care planning at the earliest opportunity in the course of memory loss and Dementia and make more readily available appropriate forms for legal and advanced care planning at no cost to consumers. Approaches will include the following: a) DAC Dementia Legal Planning Toolkit: The specialized content for a Dementia Legal Planning Toolkit is already developed and currently housed at the Northwest Justice Project (NJP). This Toolkit is a comprehensive resource with education, guidance, checklists and advance care planning forms and is available in print and in electronic form. The NJP is expected to be an ongoing voluntary partner in this work. The Contractor will work with NJP and DAC partners on annual updates and for the distribution of the Toolkit. b) Conduct Community Education and Outreach: In partnership with subject matter experts who contribute time on an in-kind basis, the Contractor will offer periodic presentations and/or participate in local resource events (at least 10 per year) for governmental and/or community organizations about legal and advance care planning for persons with dementia. The purpose of the presentations and outreach events will be to prepare community organization staff (e.g., from Area Agencies on Aging, public and/or private case management, health care, other aging network partners, etc.) with information about the Dementia Legal Planning Toolkit and tips on how to help their constituents, and to encourage and assist consumers in creating and/or finalizing legal planning documents. Partners in the development and presentation of these events will include Contractor staff and representatives of legal aid and social services organizations; and may include members of the WSBA Elder Law Section, the University of Washington Memory Brain Wellness Center, and/or the Alzheimer’s Association.
