2.4.1 Anticipated Contract Tasks The selected Proposer shall deliver services in accordance with HCBS 1915(i) and shall provide licensed and non-licensed home and community-based settings. In addition, services shall be delivered in accordance with an individual’s Individual Service Plan (ISP), as developed by County ABH staff, and delivered in a manner as to build individuals’ skills, empower mental health recovery, and increase and maximize independence. The services to be provided to individuals as a result of this solicitation include support and skills training for: 2.4.1.1 Activities of daily living (ADLs) o Personal hygiene, grooming, etc. 2.4.1.2 Instrumental activities of daily living (IADLs) o Housekeeping, laundry, managing finances, budgeting, etc. 2.4.1.3 Health and wellness o Nutrition, meal planning, arranging medical and dental appointments, diabetes and other physical health care, etc. 2.4.1.4 Medication support o Obtaining medications from the pharmacy, safe storage of medications, development of methods to foster independent medication adherence, etc. 2.4.1.5 Community integration, access, and inclusion o Scheduling medical and other transportation, utilizing public transportation, connecting with activities that support mental and physical wellness, connecting with social opportunities, etc. 2.4.1.6 Behavioral management skills o Prompting and coaching for use of crisis prevention plans, relapse prevention plans, use of coping and other skills identified in therapy, etc.
