The San Francisco Human Services Agency (SFHSA) and the Department of Disability and Aging Services (DAS) announces its intent to seek proposals from nonprofit organizations interested in providing Long-Term Care (LTC) Ombudsman Services. The LTC Ombudsman Program, as required by federal and state laws, focuses on investigating and resolving complaints from or on behalf of residents in long-term care facilities. The Ombudsman also represents resident concerns on a systems level. The grant aims to enhance the quality of life and care for residents in nursing homes, residential care homes, and assisted living facilities by: 1. Addressing complaints and concerns from residents of long-term care facilities or their representatives. 2. Offering consultation, education, and advocacy on resident rights and best care practices.
