SCOPE OF SERVICES Nyack Public School District includes three elementary schools, one middle school, one high school and one nonpublic school located within the school district boundaries. The School District Physician agrees to perform his/her duties as prescribed by the State of New York and the rules and regulations of the New York State Education Department and to serve and perform the specific duties as outlined under the Nature of Services Required and Contractual Requirements, below. The School District Physician shall be responsible for directly providing all of the medical services as outlined under Nature of Services Required and any and all services required in connection with the Nyack Union Free School District’s Committees on Preschool Special Education and Special Education, except for testimony before a Hearing Officer and/ or Court. The School District Physician Services will submit a year-end medical report, if so requested by the District. 1. NATURE OF SERVICES REQUIRED a) In compliance with New York State regulation, works with school nurses to schedule appropriate dates and to perform State mandated, in-school, physical examinations at each school of the Nyack Union Free School District for students who do not have health certificates from personal family physicians in the following classes: Pre- Kindergarten, Kindergarten, Second, Fourth, Seventh, and Tenth grades. In addition, performs physical examinations of students participating in interscholastic athletics at prescheduled sessions. These sessions occur generally on 1 date in August, 2 dates in September, November, January, March, and 3 dates in May, for a total of 12 sessions. This schedule will be finalized as soon as possible each spring, but no later than July 1 of the school year. Backup dates should also be scheduled at that time. In addition, the school physician will review examinations of athletes performed by the family physicians. b) Advises on the planning and requirements of New York State mandated immunization programs. d) Provides School Physician services at all home football games. Game schedule will be provided to School Physician as soon as available, but no later than July 1 of each year. School Physician will assign a physician to be present for each home Varsity and JV game. In the event of game schedule changes, this responsibility may be delegated to another individual previously authorized under this agreement. Staffing schedule and Physician On-Call schedules shall be provided to the Director of Pupil Personnel Services and the Athletic Director as soon as possible each year, but no later than 2 weeks prior to the scheduled athletic event, unless otherwise arranged by mutual agreement between the School Physician and the District. e) In the event the District is not satisfied with the performance of an authorized medical professional, the District shall so notify the School Physician in writing, and after receipt of said written notice, the School District Physician will refrain from utilizing the services of an individual who has been deemed unacceptable by the District. f) Administers emergency treatment when appropriate and advises school nurses in aspects of first aid.
