The County of Fresno on behalf of the Department of Behavioral Health (DBH) is requesting proposals from qualified vendors to provide an Adult Continuum of Care program. Full-Service Partnership (FSP) including Outpatient (OP)/Intensive Case Management (ICM) services for rural mental health, co-occurring, and culturally specific populations to provide comprehensive mental health, housing and community supports for adults 18 and older with a severe impairment resulting from a diagnosis of serious mental illness (SMI)/Serious Emotional Disturbance (SED) as set forth in the California Welfare and Institutions Code (WIC), section 5600.3(a). Adult Continuum of Care shall provide comprehensive mental health, housing and other community-based supports for adults.
