The Offeror shall perform the following Physician-directed services such as but not limited to: Face-to-face visits with clients’ Annual assessments /examinations Completing physician orders every 90 days Prescribing appropriate care Periodically reviewing the need for continued care Ensuring services are medically appropriate and taking professional responsibility for them Supervising client care; and Educating clients about medication effects, side effects, unusual effects, and contraindications. The Medical Doctor must provide documentation indicating the following: Personally reviewed individuals’ medical history Conducted a thorough assessment Confirmed or revised the diagnosis Initially, saw individuals face-to-face, within three sessions, or no more than thirty days of admission Reviewed and signed the plans of care; and Reviewed progress and the need for continued care every six months
