A. To provide one and/or two services through the Family Caregiver Support Program. 1. The first service being short-term solution-focused individual counseling by a qualified and licensed mental health professional. 2. The other service is support group facilitation by a trained or experienced individual. B. The target population is unpaid family caregivers who meet program eligibility for the Family Caregiver Support Program (FCSP) or the Relatives as Parents Program (RAPP). C. The County may award up to four (4) contracts for each service with the intent of reaching underserved caregivers. D. The Family Caregiver Support Program (FCSP) and Relatives as Parents Program (RAPP) aim to support unpaid caregivers during their caregiving experience so they are able to continue caring for their care partner. 1. The term “family caregiver” is used interchangeably with “unpaid caregiver” or “informal caregiver” and means an adult family member, chosen family member, friend, or another individual who is an informal provider of care in which the primary relationship is not based on a financial or professional agreement. E. The intent of the services and supports are to assist caregivers providing care and when feasible, to delay or prevent placement institutionalization and higher and more expensive levels of care.
