1. Provide a full Resident Services Program for all 48 units at the Property, which shall include at least one full-time resident services coordinator and additional resident services front desk staff, as needed, including staff to cover the front desk 10 hours per day, 7 days a week (10/7) in coordination with the property management team. 2. Work in close partnership with on-site property management, HCDD staff and SHS program team in serving residents. 3. Measure and monitor interventions, care plans and prepare monthly reports for property management, HCDD staff and the SHS program team. 4. Submit monthly invoices to HCDD for payment of services delivered. 5. Ensure data reporting requirements are met including HMIS intake, annual, and exit paperwork and data entry within timelines. 6. Provide at minimum 1.0 Full time resident services coordinator 7. Provide front desk staff to cover the front desk 10 hours per day, 7 days a week (10/7)
