Surveillance Media (CD/DVD) All surveillance media must be provided in CD/DVD format and of good, clear quality, in focus, and free of any sounds or noises. All media must include the date and time imprinted/displayed on the media during any filming activity. All locations, times, dates, and activities of the individual or business under surveillance must be documented in the body of the investigative report. All individuals that are the focus of surveillance shall be identified. This information must also be referenced in the daily investigative log. All surveillance media submitted to SIU must be properly labeled. All media labels must include: the first and last name of the individual, or complete name of the business, being videotaped; Workforce Safety and Insurance’s claim number or employer number if applicable; the dates and times of surveillance that are on the media CD/DVD; and, the total time of the CD/DVD that was used. Still Photographs Photographs submitted which contain the subject or circumstances surrounding the investigation must be clear and relevant to the investigation being conducted. All photographs submitted to SIU must be stored on a CD. All photographs on the CD must be labeled using the numeric system. The CD must be properly labeled to include the first and last name of the individual, or complete name of the business, being photographed, and Workforce Safety and Insurance’s claim number or employer number if applicable. All photographs numbered on the CD and submitted to SIU, must be accompanied with a photo log providing a detailed description of each photograph to include the identification of all individuals in the photograph who are pertinent to the investigation. These photo logs must include the first and last name of the individual, or complete name of the business, being photographed, Workforce Safety and Insurance’s claim number or employer number if applicable; the date and time the photograph was taken; the location where the photograph was taken, and investigative events/observations/commentary relating to the photograph. The investigative report must include all relevant information referring to all of the photographs submitted to SIU.
