A. The Contractor shall provide an online portal accessible to authorized DOC employees from any computer or mobile device and that can be accessed through the use of a multi-factor authentication (MFA) device or method approved by DOC Information Technology (IT). 1. DOC will provide contact information for all authorized DOC employees and those who can change authorized employees upon contract award. B. The Contractor shall enter all required information into the portal within six (6) business days of screening the enrollee. Required information includes, but is not limited to: 1. Name 2. Date of Birth 3. Social Security Number C. The portal must provide tracking information to include: 1. A database of all enrolled offenders that includes address, date of birth, Social Security Number, insurance carrier, Medicaid or private option, and policy number of each enrolled offender 2. The DOC facility at which the offender was enrolled 3. The status of each offender’s enrollment 4. The status of each offender’s application submission 5. Maintenance and backup of documentation of submission of application for three (3) years...
