Sample Tasks and Deliverables. 2.1 Administration 2.1.1 Dedicate employees with the capacity to provide the sample tasks below to the assigned former service members: 2.1.1.1 Locate the former service member and assess for program eligibility 2.1.1.2 Complete an intake with the former service member and gather documentation for their case. 2.1.1.3 Upon entry into the MNVEST Program, complete a Housing and Services Needs Assessment with the former service member. 2.1.1.4 Create and maintain participant case management file. 2.1.1.5 Enter all relevant participation data into Homeless Management Information System (HMIS). 2.1.1.6 Enter all relevant former service member data into the Homeless Veteran Registry (HVR). 2.1.1.7 Designate a staff member to attend all HVR case conferencing meetings. Meetings happen on a bi-monthly basis for each area of the state (Hennepin Continuum of Care (CoC), Ramsey CoC, Suburban Metro Area CoC (SMAC), and Greater Minnesota).
