The District is seeking qualified vendors to provide Student Transition Support Services for all SCSD schools. The District is seeking to implement a Student Transition Support Program. The Student Transition Support Program will serve students who have experienced: Long-term out of school suspension (more than five days) from school following a Superintendent’s Hearing Alternative Education Program or assignment to Homebound Instruction in SCSD Residential Placement with New York State or Onondaga County Office of Children and Family Services (OCFS) Placement in a juvenile facility (i.e.: Hillbrook Detention Center) Placement in a County or State Correctional Facility (i.e.: Onondaga County Justice Center) Other reasons as determined by the Alternative Placement Committee, District Transition Team, or Office of Student Support Services
