1. The CONTRACTOR shall have the responsibility of setting up and of obtaining genetic testing samples from persons identified by the STATE, at any times and places on all Hawaiian islands including Maui, Molokai, and Lanai as determined by the STATE. 2. The CONTRACTOR shall use only legally qualified and licensed phlebotomists to draw blood, take buccal swabs or collect tissue samples. A STATE employee will step in to perform the identical services only under extraordinary circumstances and be given a 24-hour advance notice. 3. The CONTRACTOR shall set up and obtain samples in the STATE of Hawaii, nationwide and in other countries. 4. The CONTRACTOR shall, at the time the genetic test samples are taken, properly identify the persons from whom samples are taken, and shall photograph and make a thumb print of those persons. 5. The CONTRACTOR shall provide sampling by buccal swab and/or blood draw at the discretion of the STATE. In addition, in certain circumstances, such as a deceased father, the CONTRACTOR shall conduct testing on tissue samples when requested by the STATE. Testing on buccal swab sampling and tissue sampling shall be conducted at the contract rate for regular sampling. 6. Contractor shall test samples of mother, alleged father(s) and the subject child. However, in specific cases identified by STATE, Contractor shall conduct motherless genetic testing at the contract rate for regular sampling. 7. The CONTRACTOR shall submit to the STATE within seven (7) days after the sampling, the names of the individuals who did not keep a scheduled testing appointment. 8. The CONTRACTOR shall, at its cost, provide for any redrawing and/or re-testing of samples, if and when necessary. 9. The CONTRACTOR shall ensure that any licensed phlebotomist who obtains the samples shall maintain a professional appearance at all times and wears an identification tag. 10. Should the genetic testing sample taken for another of Contractor’s cases need to be used for a new case, there shall be no additional charge to test the previously drawn genetic testing samples.
