A. PM services shall be performed semi-annually and/or annually, as detailed in Attachment A or as mutually agreed upon at the time of request. i. Semi-annual shall be defined as twice in a 12-month period, with five (5) to six (6) month intervals between PM services. ii. Annual shall be defined as once in a 12-month period. There shall be 12-month intervals between annual PM services. iii. Contractor shall provide crane services for preventative maintenance services, if needed to perform the PM service. The minimum specifications for crane services shall reach a height of up to nine (9) feet above ground level and a lifting weight capacity of at least 1000 pounds. B. PM Inspection Reports: PM services shall be documented in a PM Inspection Report, including the existing conditions and the recommended solution. i. The written PM Inspection Report shall be completed at the time of service to a system or device and shall accompany the appropriate invoice upon submittal. ii. Contractor shall provide a PM inspection report that encompasses and addresses industry-standard PM services that were performed and the results/outcome.
