This Request for Proposal for Occupational and Physical Therapy (OT & PT) Services & and Evaluations seeks to contract with individuals, companies or agencies to provide services between July 1, 2025 and June 30, 2026. Proposals submitted will be considered to provide services and evaluations for public schools students and nonpublic students via local and federal (IDEA) funding sources. OT & PT Therapy • A therapy session is defined as follows: • OT & PT – per hour (Prorated for shorter sessions). • All therapists must possess appropriate New Jersey certification. • Group sessions are considered one session and should be billed as such. No more than five (5) students can participate in one session. Group rates will not be included in proposals. • Therapy sessions to be provided on location in the recipient’s class/school; or at a recipient’s home in unique, pre-approved circumstances. • Therapists are to work around a student’s schedule. Therapy sessions are not to be provided during core curriculum classes or during breakfast, lunch, or nap time. Staff Pool List- Companies are required to provide the District with a list of employees, on a quarterly basis (October-December, January-March, April- June) as the staff pool list is a foundation of the district’s ability to claim costs in the Cost Settlement Process. Vendors that do not include all eligible staff are hindering the District’s efforts of meeting their State required end of quarter benchmark. Copies of required licenses and/or certification, contact information (phone number and email), location and monthly updates of information based upon changes of employees’ caseload is also required.
