Vendor will provide qualified licensed Registered Nurse(s) i.e., RN licensed to practice in the State of Kentucky, to complete the Minimum Data Set (MDS) and care plan process for each resident at THVC. MDS and Care Plans must be done within the guidelines established by state, federal and VA requirements using PointClickCare (PCC) electronic health records (EHR) software. To include but is not limited to: # Establish Assessment Reference Dates and coordinate Assessment Calendar # Create an online calendar that IDT (inter-disciplinary team) has access to # Utilize email to notify all IDT of assessment reference dates and any changes in assessment dates/types # Complete Admission Tracking and Open the initial MDS # Manage the calendar for OBRA Annuals and Quarterly MDSs and communicate the schedule to the interdisciplinary team # Prospective Payment System Initial and Interim Payment Assessments # Interpret and apply regulation to Interrupted Stay Situations # Track Medicare beneficiary benefit periods, qualifying hospital stay dates, and validate eligibility for skilled services # Review progress notes daily on all Medicare residents to audit for potential concerns and communicate to the designated person within the facility all identified concerns to promote prompt correction/completion # Availability during normal business hours (8:00am-4:30pm) Monday thru Friday Eastern Standard Time (EST) via telephone/email # Assist in development of user defined assessments within the Electronic Health Record to enhance MDS Coding # Assist in Restorative Nursing education # Completion of MDS, Completion of MDS Nursing Sections - Review finished MDS for completeness of all Items, provide documentation and coding training as needed, Sign as complete, Communication with onsite staff, Conference Calls / Video Conferencing to attend Medicare Meetings, Staff Meetings, Quarterly Quality Assurance and Performance Improvement (QAPI) Meetings, Resident Interviews, etc. # Transmits completed assessments to the Centers for Medicare and Medicaid Services and Veterans Affairs # Reviews validation report for proper submission and corrects any noted errors, if applicable.
