endor shall provide all labor, tools, test equipment, and materials to comprehensively perform routine scheduled inspections, preventative maintenance, repairs, and emergency services for manual and electric pallet jacks and forklifts. Models include but are not limited to: Dayton Ride-On Electric Pallet Jack – Model Number 2LEB8 & 2LEB9 Big Joe Electric Pallet Jack – Model Number E2221919 Yale Forklift – Model Number GL050RGE85.4 Landoil Bendi Series 3 B30 Forklift – Model B30/42E – 180D Eddo Electric Stacker – Model Number EB16EAS-189LI Noblelift Electric Pallet Jack – Model PTE45N Various standard Pallet Jack models Services will be performed on the following but not limited to: hydraulic system, engine, fluids, battery, body (mast, load backrest, tilt cylinder, carriage, counterweight, chassis), propane lines and connectors, steering, tires, wheels, forks, braking system, pads, rotors, lines, lights, safety measures (electrical, overhead guard, seat). The Vendor shall make repairs and perform preventative maintenance as requested to do so by a Supply Supervisor or appointed designee. All service work and parts must be preapproved prior to services being performed. Upon completion of repairs Vendor shall test run equipment in the presence of a Supply Supervisor or appointed designee to ensure the equipment is fully operational and safe to use. Vendor shall provide a repair record for each repair or call-out service rendered. The report shall include nature of call, repairs made, material used, and labor used. Reports shall state any defective equipment, instruments, and/or accessories which may require replacement or repairing. A written estimate and/or proposal for equipment repairs and parts pertaining to this contract shall include description of proposed work, breakdown of material, labor, and other costs, and will be priced as per the contract. Vendor shall provide all warranty documentation. If warranty includes labor, the facility should not be entitled to pay labor cost to replace warranted parts, materials, and/or equipment. Upon request of the Supply Supervisor or appointed designee, Vendor shall provide, remove, and/or install equipment, system controls, parts and materials related to this contract. All equipment, system controls, parts and materials must be compatible with existing equipment at the facility and must be of original manufacture or an approved equal. Contractor is responsible for properly disposing of all chemicals, equipment, system controls, and parts.
