Proposal shall demonstrate the minimum requirements for the service needed as described below. Any subcontracting of work requires prior Community approval. 1. Agency Licensing/Credentialing a. An agency submitting a proposal must have the appropriate licensing or credentialing necessary to perform the service in Arizona. 2. Licensing of Staff a. Agency staff shall be qualified as required by the type of Arizona State licensure held by the agency and/or through education, certification, and experience. 3. Case Management/Treatment Plan and Evaluation a. Each child or adult must have an assessment and any psychiatric evaluation (if applicable) in the client’s electronic health record (EHR). A discharge summary must be included for clients discharging from care. b. Clinical notes must be entered into the EHR at the time of service, and no later than 24 hours after the service. 4. Coordination with the Community a. Each service provider shall ensure clinical are coordination with the DHHS Division of Behavioral Health assigned counselor or case manager to ensure that all pertinent information is communicated. This may include individual clinical discussions, clinical staffing or team meetings. b. All individual case plans and/or treatment plans shall be coordinated with the designated DHHS Division of Behavioral Health assigned counselor or case manager. c. Providers will be expected to coordinate care with other DHHS and SRPMIC providers and staff in other areas of associated with the client’s care. d. Contracted agencies will allow the Community to bill all available third party insurances and collect for all services rendered by the psychiatrist providing services to the Community. e. Each service provider shall ensure that administrative issues and concerns are coordinated with the DHHS Administration, which will be the primary contact through the duration of the contract and for billing purposes. f. All work performed by contracted providers during their contracted time with the Community is the intellectual property of the Community. All clinical work performed will be documented in the Community’s electronic health record. 5. Compliance with Health Insurance Portability and Accountability Act (HIPAA) a. The contractor agrees to comply with the privacy rule and related standards of HIPAA outlined in Public Law 104-191 (incorporating future revisions and additions) including entering into a Business Associate Agreement with the Community. This Business Associate Agreement is required prior to entering into a contract to provide health care services to members of the SRPMIC.
