1. Complete web based test report entries on CPU owned App. 2. Provide currently calibrated test equipment needed to perform all tests as assigned. 3. Perform all scheduling and pre-notification for “call first” customers. If access to the backflow device is through private property, the contractor will notify the customer at least 24 hours in advance of the need to access the property for testing purposes. Notify Clark Public Utilities when “call first” customers do not respond to requests for appointments. 4. Notify the owner when a test has been completed by installing a door hanger informing the owner of who has performed the test and when it was completed, and if the backflow assembly passed or failed the test. 5. Adhere to the work completion requirements set forth in the Information & Instructions Section - Item 5. 6. Perform work in a manner consistent with Clark Public Utilities’ philosophies and standards. 7. Provide own transportation and tools to perform required testing. 8. Provide an email address for access to web based app and invoice processing. 9. Provide electronic device to access web based app to submit completed test reports. 10.Provide Clark Public Utilities with the name of individuals scheduled to conduct tests on private property. 11.Submit a copy of current Gauge Certification. 12.Submit a copy of current Tester Certification in Washington State.
