The organization will be contracted to: • Provide free assistance to individuals and families in navigating Minnesota Health Care Programs (MCHP). • Attempt to provide information on any other related assistance to any consumer who contacts them for help. Should client request assistance with services outside of scope of this contract, will direct the client to contact Dakota County for assistance. • Have a staff person onsite once per week for eight (8) hours, occurring between the hours of 8:00 a.m. – 4:30 p.m. Monday-Friday, at each service center, Western Service Center in Apple Valley and Northern Service Center in West St Paul, at no charge to the contract or clients served. • In addition to the 8 hours onsite, offer assistance in-person, virtual, or via telephone • Provide a monthly report capturing the following data: o Number of applications submitted including number applying for insurance o Type of application o Primary method of assistance (phone, in-person, virtual) o Use of language assistance services o Number of applications unable to assist with
