Bids must provide the following: 1. Erate SPIN Number. 2. Estimated cost for annual maintenance. a. Approximate MISSDIG items per month: 600 tickets, 20 locates b. Approximate NJUNS items per year: 50 tickets 3. Any Line item costs. 4. Any additional services not covered by USF eligibility must be itemized and quoted separately. 5. Final estimate for total annual services. Please provide pricing for one, and three years. 6. All needed coordination, supervision, labor, materials and equipment consistent and compliant with all codes and ordinances and accepted standards. 7. After hours emergency call response, able to respond to phone calls from emergency phone number that is affixed to the fiber. a. For any emergency work, provide detailed report on the outage, including photos and/or diagrams of the issue. 8. Visual inspection of the entire fiber optic network on an annual basis. Report submitted to the Director of Technology and Technical Services Supervisor. This should include identifying any network damage, needed preventative maintenance, and any areas of potential problems. 9. Routine maintenance and visual inspection, including the materials, labor and equipment. Routine maintenance should also include annual tree trimming around any potential obstructions. 10. Marker and handhole inspection and repair. 11. Read, monitor, and respond to MISSDIG, NJUNS, and any other tickets regarding fiber maintenance.
