Vendor shall perform required services using Illinois licensed professionals. At IDFPR’s request, the Vendor shall submit proof of the Illinois professionals used to perform these services. The Vendor’s total number of Illinois licensed professional staff shall be at minimum: One (1) Licensed Physician Two (2) Licensed Clinical Professional Counselors Vendor shall perform required services in facility(ies) located in the State of Illinois. At IDFPR’s request, the Vendor shall submit proof of address(es) of the facility(ies) to validate the location(s). Vendor shall maintain at minimum one (1) location. Vendor shall maintain the Illinois licensed professional(s) and facility(ies) during the entire contract term and any subsequent renewals and amendments to the contract. Vendor shall monitor and assist individual designated/mandated licensed professionals in their rehabilitation and report to IDFPR regarding the progress of those individuals.
