IDOH is interested in an LTCHIE that will support a wide variety of functions and associated services designed to support Hoosiers’ care during transitions between acute care facilities and long-term care facilities, including the ability to foster communication and access to critical patient data in a health data repository. Leveraging data exchange initiatives among long-term care facilities, hospitals, medical practices, and State agencies will strengthen public health surveillance, regulatory compliance and evidence-based policy development. Ultimately, the goal of the desired LTCHIE solution is to improve patient care outcomes. The LTCHIE should also be able to exchange data between participating long-term care facilities, acute care facilities, and with providers that share the participating acute care facilities’ EHRs and have the following capabilities: • Interoperability and secure data exchange • Ability to store patients’ health information and associated data • Ability for providers to query their patients’ health information and associated data from LTCHIE storage • Configurable user accounts/roles • Compliance with applicable regulatory standards • Scalability and adaptability to future needs
