Conduct Initial IQIP visits in accordance with the most recent CDC Operations Guide Assess provider vaccination workflow Review assessment reports and set coverage goals. Discuss/select quality improvement strategies and provide technical assistance Establish action items for the strategy implementation plan. Enter data into IQIP Database Conduct 2-month IQIP visits in accordance with the most recent CDC Operations Guide Prepare by reviewing synopsis and notes from initial site visit. Review strategy implementation plan and discuss implementation status Identify barriers and provide technical assistance. Establish new action items for updated strategy implementation plan, if necessary. Enter data into IQIP Database. Conduct 6-month IQIP visits in accordance with the most recent CDC Operations Guide (6-month visit can be conducted virtually). Prepare by reviewing synopsis and notes from 2-month check-in. Review strategy implementation plan and discuss implementation status. Identify barriers and provide technical assistance. Establish new action items for updated strategy implementation plan, if necessary. Enter data into IQIP Database. Document all visits in the CDC IQIP database (RedCAP) within 10 business days of the visit as required in the CDC’S Operation Guide. Send completed site visit reports to DPH via the CDC IQIP database (RedCAP).
