The City is seeking to establish one (1) or more contracts with qualified medical providers for post-offer and periodic medical examinations/medical surveillance for current and future employees. This document constitutes an RFP from qualified individuals, firms, or organizations to provide health and medical services as described herein. The purpose of this RFP is to conduct a competitive process to create a contract(s) from which the City may acquire these services. When preparing a Proposal, Contractors should specifically identify their capability to: 1. Have a minimum of one (1) worksite located in Flagstaff, Arizona, with standard business hours and a minimum four (4) day per week service. 2. Provide the personnel necessary to provide the services contained within the Scope of Work. 3. Provide related written reports to the City within three (3) business days of service or of request by the City. 4. Provide a reference for two (2) current contracts where post-offer and periodic medical examinations/medical surveillance are a component of that contract.
