This Survey should be prepared as a Consensus Determination of Eligibility (CDOE) document. The CDOE will form the basis for identifying historic resources and evaluating the district for listing in the National Register. The CDOE should be formatted as a National Register Nomination and prepared by a professional who meets the Secretary of the Interior’s Professional Qualification Standards in History or Architectural History. The CDOE should contain the following: • National Register nomination form (all sections) • Historic images (e.g., maps, photos, postcards) illustrate the village’s development over time. • Current photos (individual photos of highly significant buildings, and representative streetscapes) • Locational map (topo, Google Earth or similar) • Map showing historic district boundary, lot lines, and street addresses, and indicating the contributing/non-contributing status of each resource in the historic district.
