Contractor will be responsible for performing the work according to the following specifications: 1) Submittals – Contractor will submit the following: a. Site specific SOW that details the coating removal strategy, recoat plan (if any), construction and maintenance of any required enclosures, hazardous material venting considerations and waste material handling. b. Hazardous Waste Disposal Plan: The plan will include debris and hazardous waste removal, recovery, and disposal methods, if necessary. The plan will include prevention methods to prevent hazardous waste from entering surface and ground waters, if applicable. An independent Certified Industrial Hygienist (CIH) will prepare the Hazardous Waste Disposal Plan, as well as any sampling and removal plans. c. Name, location, and license number of the refuse site authorized to accept the debris and hazardous waste to be removed from the work sites. d. Name and permit number of transporting agent(s) to transport the debris and hazardous waste to the refuse site. e. Hazardous Waste Transport Plan: Plan will indicate route from work site to disposal site. f. Hazardous Waste Shipment Documentation: Provide copies of the transportation manifests or bills of lading. g. A list of materials required for the job with Safety Data Sheets (SDS). h. Hazardous waste sampling and testing reports. i. Site Safety Plan for personnel involved in hazardous waste removal, recovery, and disposal methods. Contractor will provide a Health and Safety Plan specific to DWR requirements. This document will be reviewed and compared to DWR protocols. Plan will be submitted to DWR three (3) weeks prior to work for review. This Pre-Work Safety Plan will be required on-site along with a daily hazard assessment. j. Provide certificates for the lead competent person training, asbestos awareness training, asbestos inspector training as applicable, and a list of first aid/CPR trained employees with documents that support the training is current to Field Division Safety staff prior to start of work. k. Contractor will provide ventilation plans with engineered ventilation calculations to ensure that adequate ventilation is being provided.
