SITE SURVEY All Service Companies desiring to provide services specified shall visit the location listed above to become familiar with the facility and equipment prior to submitting a proposal. No proposal shall be accepted from any Service Company unless it has been verified that there has been a site visit prior to submittal. The Service Company shall not be relieved from assuming all responsibility for properly estimating the difficulties and the cost of performing the services required with this specification, because of failure to investigate the conditions or familiarity with information concerning the services to be performed. The mandatory prebid meeting will be held on January 9, 2025, 11:00AM at the Larned State Correctional Facility, 1318 Kansas Highway 264, Larned, KS 67550. SERVICE COMPANY QUALIFICATIONS The following requirements shall be considered as the minimum standards for Service Company to be considered as qualified to provide services under this contract and shall be a pre-requisite to any award. • A period of TEN (10) years’ experience in the performance of boring, trenching and installation of the Poly Pipe and Vaults, as specified in the local geographic area shall be considered a minimum. • The Service Company shall maintain a field office within a reasonable distance of the facility to be serviced under this specification. • All qualified technicians shall have passed previous background checks and can work in secure facilities. • Services that are to be provided shall be performed by qualified and trained service personnel that are directly employed by the Service Company. Sub-contracting portions of the system or services requested in this specification shall not be allowed without prior consent. • The Service Company shall have a designated quality assurance contact that will meet the site manager before and after the scope of the service contract have been completed, to ensure services needs are satisfied. The quality assurance contact shall review the agreement performance to date as well as make any changes necessary to improve the quality of services delivered. SERVICE COMPANY RESPONSIBILITIES / SCOPE OF SERVICE This contract will provide the placement of two (2) service company supplied, by bore and trench 1.25 poly lines of approximately 2760 feet. The service company will place owner supplied optic cable, approximately 3600 feet. The service company will also provide, and place three (3) service company supplied pull vaults. Service company will enter existing vaults and buildings. The service company activities will be scheduled with the owners designated personal. Service company is required to survey and mark all underground utilities, this is to be coordinated with the designated personal. Alternat #1 The service company will supply the 36-strand single mode fiber. SERVICE COMPANY SAFETY REQUIREMENTS To be considered qualified to perform under this Request for Proposal, the Service Company must be able to demonstrate a robust safety program to include: A robust training curriculum that includes, but is not limited to, (1) Confined Space Entry (2) OSHA Trenching and Excavation Standards: 29CFR 1926.650, 29 CFR 1926. 651, 29 CFR 1926. 652 (3) Personal Protective Equipment (4) Hazard Communications (5) Health Hazards (6) Blood Borne Pathogens (7) Job Equipment Specific Training Etc. A one-year warranty for pipe damaged during placement. A sight visit is required during the Prebid meeting.
