The McHenry County Department of Health is seeking to contract with a qualified firm to perform a community health needs assessment, subject to continuing need and availability of funds. The assessment will include three components: survey, focus groups and key informant survey. Final reports of all components will be compiled. The County expects to have approval and would like to begin the process in February 2025, having all reports complete by August 2025, and release of results in 2026. 1. Complete data analysis of all components of the study 2. Raw data submitted to MCDH after project completion 3. Community survey — representative sample of McHenry County residents a. Supply MCDH with a csv containing the statistics and margin of error for all indicators. A template will be provided by MCDH b. Calculate statistics for the county and specific demographic groups, and where possible, compare the data from the current survey to the historical data 4. Focus Groups — 3 total 5. Key informant survey — contacts for survey will be provided by MCDH 6. Partner agencies shall be invoiced individually by the consultant
