• Purchasing disability replacement parts. • Purchasing high-demand disability equipment (i.e., bariatric equipment, manual wheelchairs, etc.) • The provider must have storage for Durable Medical Equipment. • Provide Durable Medical Equipment to citizens within 72 hours of request. • The provider must have policies and procedures that encourage responsiveness to all requests in a timely manner and track when equipment is returned or scheduled to be returned or extended requests. • The providers must submit invoices within one week of purchase of durable medical equipment. • The provider must be able to deliver or distribute durable medical equipment to citizens within Duval County or arrange for citizens to pick up equipment. • The provider must keep efficient and accurate records of citizens receiving durable medical equipment, which is a key responsibility. This includes client name, contact information, address, type of equipment received, and the value of the equipment. • The provider must keep an inventory of all durable medical equipment purchased with this funding and the cost of each piece of medical equipment. • The provider has a current program that offers durable medical equipment on loan or has a loan closet for durable medical equipment to ensure continuous access to necessary equipment. • The Durable Medical Equipment program assists citizens within Duval County in ensuring the resources are used for the benefit of the local community. • The provider must be able to provide placement parts for equipment and maintain the following equipment in inventory (manual wheelchairs, rollators, shower chairs, and various bariatric equipment). • The provider must provide a quarterly report of the number of citizens serviced, equipment purchased, and amount.
