The vendor is able to work with the ISU and Ames Lab Occ Med Services department to provide services necessary for the sustainment of a program of medical surveillance for ISU and Ames Lab employees exposed to workplace hazards as required by federal and state regulatory requirements as well as requirements of the Ames Lab contract with DOE. The vendor must provide the following services: Medical consultation on employee issues or concerns of work-related exposure risks, including symptoms of exposure Medical examinations (initial, annual or exit) by a licensed medical professional(s) Medical support for programs affiliated with the Office of Research Ethics (ORE), such as the Institutional Review Board (IRB), the Institutional Biosafety Committee (IBC), the Institutional Animal Care and Use Committee (IACUC) and the Radiation Safety Committee (RSC), as well as other accrediting bodies. Conduct Pre-employment and Termination Physicals Treatment of minor occupational injuries and illnesses and referral of more significant cases to outside medical facilities Refer employees to a private physician for medical conditions that are unrelated to workplace exposures. Provide services listed in 10 CFR, Part 851, Appendix A for Ames Lab with the assistance of Ames Lab Environmental Safety and Health Department Coordinate with ISU and Ames Lab on the participation and medical surveillance of employees in the following programs: Biosafety Program Bloodborne Pathogens Program Chemical Hygiene Program Hearing Conservation Program Laser Safety Program Radiation Safety Program Respiratory Protection Program Ames Lab Beryllium Registry Other programs requiring medical surveillance
