Services shall include but is not limited to: 1. Rehabilitative Services: for the purposes of this Agreement, rehabilitation Services are those services the contractor shall provide at the Home. Rehabilitation Services shall be provided in accordance with Medicare and Medi-Cal guidelines and the Home’s policies and procedures. The contractor shall ensure certified, licensed and experienced staff provides these services. 2. Physical, Occupational and Speech Therapy Services: Contractor shall provide Rehabilitative Services to include Physical Therapy, Physical Therapy Assistants, Occupational Therapy, Occupational Therapy Assistants and Speech Therapy Services as authorized by the Home. These services shall be provided at the Home on a scheduled and as-needed basis. a) Contractor shall provide appropriately certified and trained (as defined by 22CCR) rehabilitation therapists and other affiliated providers ("Therapists"), who shall provide scheduled and as- needed services for the Home. Therapists assigned to the Home must have at least a year of SNF experience. b) Following advance coordination, contractor shall provide to the Home’s Chief, Restorative Care Services, a weekly schedule setting forth the type of Rehabilitative Services to be rendered, the name of the Resident(s) to receive services, and the time schedule for services to be provided at the Home. This schedule is to be provided to the Home at least one (1) calendar week in advance. Patient load will determine hours of service and staff required to respond to those needs.
