Homeless Trust Fund assistance to eligible clients referred by the County Human Services Division of Homeless and County Social Services (having been determined as homeless or atrisk and not eligible for Emergency Assistance), and other Homeless Serving agencies including self- referral. Agency will provide outreach and public information regarding funded services, including, but not limited to announcements, flyers, newsletters, referrals, or media releases. Written procedures for handling requests for assistance within the agency are to be maintained and are to include procedures from receipt of inquiry to follow- up on client status after receipt of assistance. Provide follow-up upon discharge to determine client’s progress, status or need for additional assistance when the ability to contact the client exists. This must be documented in HMIS as Case Notes.
