The scope of work shall include but may not be limited to bringing in person Spanish and English behavioral health services for Lake County children, families and adults who may be or at-risk of being system involved. System involved includes but is not limited to: individuals involved in the criminal or juvenile justice system court system, child welfare, or adult protective services. Expectations include: • Conducting group and individual therapy; while also collaborating with families, child welfare, law enforcement, courts, parole officers and others, depending on the circumstances; • Develop individualized treatment plans to address substance use disorder and other related needs; • Adjusting treatment needs depending on patients' progress; • Help patients maintain their recovery; • Keep documentation to track the progress of treatment; • Evaluate and refine treatment plans on an ongoing basis.
