Individuals/Businesses submitting a proposal should review the procurement requirements listed. Specifically, the selected Individual/Business will be required to: • Execute a contract with Woods Cross City. • Maintain General Liability Insurance ($1,000,000 minimum), Workers' Compensation Insurance ($500,000), and business automobile liability ($1,000,000) and furnish proof of such insurance. List Woods Cross City as additionally insured and as the certificate holder on the Certificate of Liability form for Workers Compensation • Be a State of Utah Licensed and bonded Contractor • Be a State of Utah licensed Contractor or Subcontractor for Asbestos Abatement and follow the Environmental Protection Agency (EPA) and the State of Utah regulations for Asbestos abatement. All employees must follow OSHA requirements to protect themselves and the public. See the attached Asbestos report for reference and required abatement. • Haul asbestos materials and dispose of in a State of Utah and US EPA-approved disposal facility • Dispose of building debris in a State of Utah and EPA-approved facility • Obtain and pay for all permits and costs required for Asbestos Abatement, any inspections, and demolition permits with federal, state and local authorities (City Demolition permit Fee waived).
