1. Contractor will work with qualified individuals and community centers who do not have technology necessary to allow them to participate in secure, private conversations with their doctors, mental health counselors, family members, etc. 2. Contractor shall provide the following services to reduce barriers to social isolation and loneliness: a. Application Process: Contractor will develop a consumer intake application based on participant eligibility criteria provided by Washington County. b. Managing Intake and Applicant Information: Contractor will manage intake and applicant information in the same secure manner as authorized and approved for state and federal grant activities operated by the agency. c. Assessment: Contractor will conduct a remote assessment by a qualified assessor from to determine specific equipment needs and recommendations as well as training requirements. Some participants may require additional accessories to use the tablet or lightweight laptop due to disabilities. Should an individual require additional accessories due to disabilities, contractor will contact County to discuss additional costs associated with these disability accommodations. Accessories are not expected to exceed $300 per participant and when necessary, will be provided after the individual receives their tablet or computer. Service Rate: $275 per qualified applicant...
