1.1. SUPPLIES AND/OR SERVICES REQUIRED: The Illinois State Toll Highway Authority (Tollway, Illinois Tollway or ISTHA) is seeking a vendor to provide the following health screenings at its Health Fairs: cholesterol and glucose screenings, blood pressure monitoring and flu vaccinations. 1.1.1. The Vendor must supply all equipment and personnel to perform the above screenings. The Health Fairs are typically held in October and/or November at various ISTHA locations (See Section 1.4), in accordance with ISTHA and Vendor’s schedules. The Tollway may/may not opt to hold Health Fairs for all locations. 1.1.2. Vendor shall take all necessary measures as required by all applicable federal and/or state laws, rules, and regulations to protect the privacy of the employees who participate in the Health Fairs and ensure against the unauthorized release of protected information. 1.1.3. The Vendor shall provide an information phone number for employees to call if they have any questions or concerns after they receive their results and shall be active for a minimum of thirty (30) days after all Health Fairs are conducted. 1.1.4. The Vendor shall be current and remain up to date regarding legal and compliance issues surrounding health screenings, and product viability and availability issues as well. 1.1.5. The vendor shall be able to handle requests that deal with socio-economic environments and cultures.
