A. Furnish all labor, equipment, tools, materials, parts, and supplies to perform all repair and maintenance services specified herein; B. Repair, upkeep, and maintain all dental equipment to reduce downtime at the following locations: Lanakila Health Center, Dental Clinic: 1700 Lanakila Avenue, Room #202, Honolulu, Hawaii 96817 Leeward Health Center, Dental Clinic: 860 Fourth Street, Room #250, Pearl City, Hawaii 96782 Windward Health Center, Dental Clinic: 45-691 Keaahala Road, Building D, Kaneohe, Hawaii 96744 Hawaii State Hospital, Dental Clinic: 45-710 Keaahala Road, Kaneohe, Hawaii 96744 Diamond Head Health Center, Dental Clinic: 3627 Kilauea Avenue, Rm. #403, Honolulu, Hawaii 96816 C. Maintain and provide adequate number of technicians during the normal business hours of Monday through Friday from 7:45 a.m. to 4:30 p.m. except weekends and STATE holidays to ensure timely repair. The STATE holiday schedule is attached hereto as Exhibit “A” and made a part hereof; D. Deploy a technician to the dental clinic(s) as needed or as requested by the STATE. E. Purchase parts and/or materials necessary to repair and maintain the equipment; and F. Coordinate with the STATE to schedule a repair and/or maintenance service as soon as the CONTRACTOR receives the necessary parts and/or materials or a technician(s)’ schedule is determined.
