Scope of Work/Services The Mat-Su Health Foundation seeks proposals from experienced evaluators to create and implement a renewed evaluation plan for the Youth 360 initiative. • The evaluation plan should be developed through meetings with program staff and key stakeholders such as leadership, partners, youth participants, and other initiative consultants. • Develop data collection forms and measurement tools that are identified as needed by the evaluation plan to create a baseline measurement. • Collect program-specific data that reflects activities and outputs. 3 • Identify and incorporate a regular schedule of feedback and learning. • Prepare annual summary reports and one final report. • Participate in regular check-in meetings with the MSHF and Youth 360 leadership. • Visit Youth 360 Club Houses and participate in the Youth 360 data and evaluation workgroup
