1. Services provided for programs in this service area could include the following: a. Intensive, wrap-around, trauma-informed case management services b. Financial Assistance, including: i. Application fees ii. Deposit assistance iii. Rental assistance iv. Utility assistance v. Barrier elimination costs 2. Deliverables: Reporting requirements for programs in this service area would correspond with service costs, as listed above. These could include: a. Financial assistance dollar and clients totals b. Number of clients provided case management c. Client demographic information For House 20 programs, this will also include the following deliverables: i. 20+ individuals will receive housing location and case management services ii. 3-5 individuals annually will successfully exit the program into permanent supportive housing
