The Consultant will use the results of the recently completed CHA to guide in the development of a CHIP. Expectations include, but may not be limited to: 1. Develop a comprehensive CHIP leveraging the existing CHA; align the CHIP with state and federal health objectives and frameworks. 2. Thoroughly review the completed CHA to understand identified health issues, community needs, and priorities. Extract and summarize key data points and findings relevant to the CHIP; prioritize health issues based on prevalence, impact, and community concern. 3. Address community health needs, priorities and gaps identified in the CHA. Identify any additional data needs or gaps not covered in the CHA that may impact the CHIP development. 4. Organize and facilitate visioning sessions with community partners and stakeholders to utilize CHA in the development of the CHIP. Define roles and responsibilities for implementation among stakeholders and community partners. Must be based on Mobilizing for Action through Planning and Partnerships (MAPP) model or other approved model by the health department. PHAB Domain 5 Standards must be met.
