Lease Agreement for the All- in-One, Water & Ice Dispensing Machines (“Machine”). The suppler shall be responsible for including the following services in the Total Annual Price. • Transporting, completing installation, set up to include connection of the water supply, establishing power and/or everything necessary to have the machine running upon installation. • Monthly and/or as needed inspections, all maintenance and repairs, including manufacturers recommended filter replacement. • Maintain all manufacturer recommended updates, replacements and any warranty issues that may occur with the Machine. • Shall respond to all Service requests within two (2) consecutive calendar days of notification and must complete the repair or replace the Machine within three (3) consecutive calendar days from the initial receipt of the Service request. Service requests can be submitted through email or phone call. • Supplier shall be responsible for any damages caused by the maintenance or improper installation of the Machine that may lead to leaks. • Removal of the Machine, equipment and site restoration, once the agreement has completed. No later than fifteen calendar days upon the completion or termination of the lease. • *All equipment shall remain the property of the awarded Supplier throughout the Lease. *
