EOHHS is issuing this RFA to solicit applications from eligible ENS Vendor Applicants to become Certified ENS Vendors that will form the Statewide ENS Framework. This procurement will set out the certification criteria for how eligible ENS Vendor Applicants become certified by EOHHS. This document and the accompanying attachments will also set the obligations, including the applicable terms and conditions program participants are required to agree to, of the Certified ENS Vendors. Attachment A, Statement of Work (which incorporates Attachments B and C), is the Contract that will be awarded pursuant to this RFA. The applicant’s response must recite that the applicant accepts the provisions of Attachment A (which also incorporates Attachments B and C), without revision. The Applicant shall not include any statement in any portion of its response to this RFA, that conflicts with this acceptance, such as a purported “reservation of rights” or “conditional acceptance” of the terms of the Contract.
