Contractor will provide MSVA with an invoice for veteran residents who have been rated as in need of nursing home care for a VA adjudicated service-connected disability; or the veteran has a singular or combined service-connected rating of seventy percent (70%) or more based on one or more service-connected disabilities or a rating of total (100%) disability based on individual unemployability. Contractor agrees that for all other residents, provider will first bill Medicare for laboratory services performed for residents that have Medicare Part B. Provider further agrees to bill any third-party insurance for amount not covered by Medicare for residents with coverage. Contractor will invoice Residents for any and all laboratory related cost which are not covered/reimbursed by Medicare or third-party insurance carriers and for those that has no insurance. MSVA is not responsible for any payment owed. Contractor shall be certified by the Clinical Association of Pathologists (CAP) and/or Clinical Lab Improvement Amendments (CLIA) and shall provide documentation of certification with proposal. Contractor shall provide a qualified person to perform all routine lab draws daily on Monday through Friday. No routine lab draws should be performed before 5:00 A. M. Contractor shall provide transportation of all samples. Samples should be picked up daily Monday through Friday no later than 10:00 A.M. Contractor shall provide a schedule of turn-around times for all test results and any required test results documentation to the HOME. Contractor shall provide collection supplies for labs other than routine to be collected by staff of the HOME. Contractor will conduct required chart audits only when requested by MSVA at a charge of $75.00 per hour. Contractor shall provide for all test results to be transmitted electronically to a dedicated printer, if required by the Provider’s proposed information system. Contactor must notify facility verbally of any of critical labs. In addition to the requirements of the Record Retention and Access to Records clause, Contractor agrees to maintain records all lab tests and results for a period of three years and make said records available to the MSVA upon request. Contractor will provide tubes when necessary to run labs. Contractor will provide the facility with portal access for all lab results. Contractor will provide STAT services seven (7) days a week with Saturday and Sunday orders “On Call” 8:00 A.M. - 6:00 P.M. Providing laboratory testing for residents of Client for the following testing classifications: General Chemistry and Hematology Testing, PCR Testing and Toxicology.
