The Department of Juvenile Justice (DJJ) and the Office of Probation and Community Intervention are issuing this RFP for an Aftercare Resource Navigator Program (Program) in each judicial circuit. The purpose of the Program is to increase positive outcomes for youth who are reentering the community from a residential commitment facility (Facility) by identifying and establishing relationships with resources that address the service needs of youth and families, connecting youth served by the Program and their families with the appropriate resources to address their specific service needs, and collaborating with Department staff, Facility staff, educational and workforce credentialing providers, other targeted service providers, community partners, and stakeholders. The Program shall provide transitional planning services to youth who are pending release from a Facility, including youth who will be supervised on Conditional Release (CR) or Post-Commitment Probation (PCP) upon their release, and Direct Discharge youth, who will be released with no supervision. The Program shall continue to provide services for a period of time after the youth’s release from the Facility to ensure that services identified during transitional planning are completed or substantially underway.
